FSAM Registration


REGISTRATION FOR FSAM MANUFACTURERS PRODUCING CAAS GVS CERTIFIED VEHICLES

Final Stage Ambulance Manufacturers (FSAM) companies producing CAAS GVS certified vehicles are required to register with CAAS GVS in order to obtain the GVS certification stickers and verification documents required to certify these vehicles.

CAAS GVS provides these stickers and associated document templates to FSAM companies who attest that they will comply with all CAAS GVS verification and certification requirements for vehicles they produce. FSAMs applying for registration with CAAS GVS will complete the FSAM Registration Form including detail on certifications they have acquired in order to comply with the standard. An officer of the company must sign both pages of the document verifying the data submitted as correct and complete. CAAS GVS provides no recommendation or validation of any ambulance manufacturer or their certifications, and cannot be held responsible for any discrepancy, misunderstanding or misrepresentation by a FSAM. Furthermore, CAAS provides no warranty of any kind relating to CAAS GVS certified vehicles or any ambulance services.

The FSAM is required to certify each vehicle as compliant with CAAS GVS. The FSAM is obligated to certify to the purchaser that the ambulance bearing the “CAAS GVS” label, its components and equipment meet or exceed all the requirements and tests set forth in the standard. VEHICLE CERTIFICATION PACKAGE – GVS Certification requires three documents to verify compliance for each vehicle:

  1. A serialized multicolored CAAS GVS Certification Sticker installed on the vehicle. This is a two part sticker that will come to you already imprinted with your name as the registered FSAM, and serialized to your company. A separate template will allow you to enter specific vehicle data in a window that will be visible to the customer and inspector when the sticker is applied to a GVS certified vehicle.
  2. A multicolored Vehicle Compliance and Exception/Variance Verification electronic template document (per Appendix 1, Figure 2) to be completed, printed and signed by the FSAM. This document shall be provided in the handbook of instructions.
  3. A multicolored Payload Calculation and Verification electronic template document (per Appendix 1, Figure 3) to be completed, printed and signed by the FSAM. This document shall be provided in the handbook of instructions.

FSAM companies must fill out this FSAM Registration Form and submit to CAAS GVS as noted. Both pages of the form must be signed by an officer of the company verifying that the information provided is correct, and agreeing to comply with the testing and certification requirements of the standard. Following review and approval, you will receive access to the online storefront to purchase the Vehicle Certification Packages outlined above.

Please note: A $1000 registration fee for FSAMs and Remounters is required by CAAS-GVS as of January 1, 2024.  This fee will be charged as part of the official  application approval process.
